Management of III° open fractures of the leg. Background: About one third of all patients with fractures of the leg have to be classified as open fractures and requires operative treatment including bone and soft tissue repair. Method: In a retrospective study the management of 53 patients with open fractures of the leg admitted to our hospital was analysed. Results: Among them, 43 patients were operated on: 37 received an external fixator; 4 debridement only and two legs had to be amputated. Three patients sustained a defect fracture; 12 patients presented with a soft tissue defect following the initial debridement. Of those 12 cases, nine wounds got primary or secondary infected, but could subsequently covered with a muscle (n=7) or skin flap (n=3) or healed spontaneously. Conclusion: Despite the existing principles of surgery of open fractures, the management of III° open fractures of the leg needs to be modified according to the individual type of injury. |
